Adult Release Form

You must sign this form if you are not a Sunset H.O.G. member. This includes guests of members or their passengers. You can save time before the ride and fill it out online now, print it, sign it, and bring it with you to the ride. It's much easier to do it now than to wait until ten minutes before the ride, and then try to find a pen while everyone else is listening to the ride instructions.

Minor Release Form

The parent or guardian of anyone under 18 who is riding with us must sign this form before the ride. A witness must also sign. Again, it's very easy to click on the above link, download the form to your computer, open it in Acrobat, type in all the info into the fields, print it, sign it, and bring it with you to the ride.
